Affordable Care Act (ACA) Form 1095-C Notice

Businessolver, which provides benefit administration services to CUA, mailed the Form 1095-C to your home address on record. The 1095-C provides medical plan information needed for your 2015 tax filing. You can also access your Form 1095-C electronically on the CUA benefit administration website at mycuabenefits.com.

More detailed information was provided in the February edition of Inside CUA. If you have questions specific to your CUA health care coverage, email Debby Lee at leedj@cua.edu.

Important Deadlines for Flexible Spending Accounts and Vacation Usage

  • 2015 Medical and Dependent Care Flexible Spending Accounts (FSA)
    If you have a remaining 2015 balance, you may still incur expenses against your 2015 account(s) until March 15, 2016.

    The deadline for submitting your 2015 claim(s) is April 30, 2016. To submit your 2015 claim(s), please complete the Medical FSA and/or Dependent Care FSA forms and submit them directly to the Health Care Account Service Center at the address located on the forms.
     
  • Annual Leave (Vacation) Policy
    The annual leave policy is based on the fiscal year, May 1 to April 30. We encourage you to use your appropriate annual leave time before April 30, 2016, or you may lose vacation time.

    Exempt employees can carry over up to a maximum of 21 days into the next vacation year, beginning May 1. All carry over vacation days in excess of seven days must be used by Aug. 15 within the new vacation year.

    Non-exempt employees hired before Jan. 1, 1994, may carry over a maximum of 30 days of accrued vacation leave to the next vacation year, beginning May 1. Non-exempt employees hired on or after Jan. 1, 1994, may carry over a maximum of 21 days of accrued vacation leave to the next vacation year.

    You can access your vacation hours balance on your most recent paycheck. Please submit all leave reports and time sheets regularly with each payroll cycle to ensure that your time-off records are accurate.

New UnitedHealthCare (UHC) Online Tools

There are two new online tools for UHC-covered members:

UHC Rally — Rally is a personalized web portal to help users make healthier lifestyle choices. It replaces the current Health & Wellness portal on the UHC website. Enroll and complete an online assessment to receive a $25 gift card!

UHC Virtual Visits — Employees may now visit a doctor “virtually” via webcam on a mobile device or computer. Doctors are available 24 hours a day, 365 days of the year. Please log in to myuhc.com to register for a virtual visit.

2016 Monthly Retirement/TIAA-CREF Counseling Sessions

A TIAA-CREF representative will be on campus to provide one-on-one retirement information sessions for employees from 8:30 a.m. to 4:30 p.m. The sessions are free, but you must call the Meeting Request Center number at 800-732-8353 to schedule an appointment for one of the below dates. You may also attend sessions provided in Spanish.
